Who Qualifies for Proactive Health Benefits?
All company employees earning a W-2 income are eligible to apply for Proactive Health Benefits. Employees earning at least $22,000 per year will experience increased take-home pay, but all employees are welcome to participate.

How Long Will This Endure?
PHB’s Comprehensive Health Solution, powered by Proactive Health Benefits, includes a unique indemnity insurance plan that offers financial protection to employees in the event of a qualified medical occurrence. This fully-insured limited benefit plan safeguards individuals through indemnity insurance claim payments.
The plan’s premiums are managed through an employer group’s Section 125 cafeteria plan, enabling pre-tax contributions via salary deductions, in compliance with the IRS Code. This structure provides financial security and tax advantages, making it a cost-effective solution for both employers and employees.
The Section 125 cafeteria plan provisions were introduced by the United States Congress as part of the Revenue Act of 1978. This legislation allows employees to choose between receiving cash compensation or certain qualifying benefits (such as health insurance and life insurance) on a pre-tax basis. Importantly, there is no sunset date for this program, ensuring its ongoing availability.
